Immediate Attention for PIs & Clinical Coordinators Regarding Obtaining Hospital & Clinic Account Number
Please note that the procedure for requesting a clinical trial account from Tulane University & Clinic has changed. In order to receive an account number, Tulane University’s Office of Sponsored Projects Administration (SPA) must first issue a work order to Tulane hospital. Effective immediately, the following forms must be submitted to SPA:
- Department cover letter
- IRB-approved protocol
- IRB-approved informed consent/assent
- IRB approval letter
- TUHC forms. These forms can be found here. See Forms for HCA & also the applicable forms under DUA forms.
Forms may be emailed to vernalee@tulane.edu, faxed to Verna at 988-1748 or delivered to SPA’s office at Elk Place, Suite 201.
Once SPA has these documents, it will prepare and issue a work order to the hospital. Once the hospital has signed the work order, the hospital will then contact the PI/coordinator to provide an account number.
